Pharmachologic effect

The combined drug. Metamizole sodium is a derivative of pyrazolone and has an analgesic, antipyretic and mild anti-inflammatory effect, the mechanism of which is associated with inhibition of prostaglandin synthesis due to inhibition of COX.

Quinine depresses the center of thermoregulation, in high doses - the auditory and visual zones of the cerebral hemispheres. Quinine also inhibits the development of erythrocyte forms of schizonts and gamonts of malaria, inhibiting DNA replication, and has a local irritating and local anesthetic effect.

The pharmacological effect develops 20-40 minutes after taking the drug and reaches a maximum after 2 hours.

Indications

- pain syndrome of various etiologies (including headache, neuralgia, arthralgia, myalgia, myositis, algomenorrhea, postoperative pain);

- renal and biliary colic (in combination with antispasmodics);

- febrile conditions for infectious and inflammatory diseases.

Dosage and administration

Adults and children over 14 years of age, the drug is prescribed 1-2 tablets. 1-3 times / day depending on the clinical picture. Children aged 12-14 years - 1 tab. 3 times / day

The duration of taking the drug (without consulting a doctor) as an antipyretic is not more than 3 days, as an analgesic, not more than 5 days.

Tablets should be taken without chewing, after eating.

Side effects

From the digestive system: nausea, vomiting, pain and discomfort in the epigastric region and the abdomen, diarrhea; rarely - ulceration of the gastrointestinal mucosa, gastrointestinal bleeding.

From the side of the central nervous system and peripheral nervous system: headache, dizziness, fatigue, insomnia, tremor of the extremities.

On the part of the sensory organs: tinnitus and hearing loss, impaired vision (blurred vision, scotoma, photophobia, diplopia, decreased field of view, impaired color vision).

From the cardiovascular system: angina pectoris, palpitations, fainting.

From the hemopoietic system: leukopenia, agranulocytosis, hypoprothrombinemia.

From the urinary system: impaired renal function (including oliguria, anuria, proteinuria), water and electrolyte retention.

Allergic reactions: sometimes - itching, eczema, enanthema, urticaria, angioedema; rarely - anaphylactic shock; in some cases, malignant exudative erythema (Stevens-Johnson syndrome), toxic epidermal necrolysis (Lyell syndrome), bronchospastic syndrome.

Other: fever.

Contraindications

- severe violations of the liver;

- severe renal impairment;

- bronchial asthma;

- asthma caused by taking salicylates or other NSAIDs;

- diseases accompanied by bronchospasm;

- hereditary hemolytic anemia caused by deficiency of glucose-6-phosphate dehydrogenase;

- inhibition of hematopoiesis (agranulocytosis, neutropenia, leukopenia);

- severe myasthenia gravis;

- diseases of the middle and inner ear, hearing loss;

- optic neuritis;

- children's age up to 12 years;

- Hypersensitivity to the components of the drug, as well as to other pyrazolone derivatives.

With caution , the drug should be prescribed with hypersensitivity to other NSAIDs, with food, drug or other forms of allergies (including hay fever, bronchial asthma), granulocytopenia (monitoring of hematological parameters is necessary.)

Interaction with other drugs

The simultaneous use of metamizole sodium with other antipyretic analgesics can lead to a mutual increase in toxic effects.

Tricyclic antidepressants, oral contraceptives, allopurinol disrupt the metabolism of metamizole sodium in the liver and increase its toxicity.

Barbiturates and phenylbutazone weaken the effect of metamizole sodium.

Sedatives and tranquilizers enhance the analgesic effect of metamizole sodium.

Metamizole sodium enhances the action of ethanol-containing drinks.

Metamizole sodium, displacing oral hypoglycemic drugs, indirect anticoagulants, corticosteroids and indomethacin from the protein bond, enhances their effect.

Quinine can potentiate the effect of drugs that block neuromuscular transmission.

Quinine increases the concentration of digoxin in blood plasma and enhances its effect.

Cimetidine reduces clearance and increases T 1/2 quinine.

The simultaneous use of Analgin-Quinine with cyclosporine reduces the level of the latter in the blood.

Microsomal oxidation inducers accelerate metabolism and reduce the effectiveness of the drug; microsomal oxidation inhibitors increase the effectiveness and risk of side effects.

With simultaneous use with tiamazole and sarcolisin, the risk of developing leukopenia increases.

Codeine, propranolol enhance the action of Analgin-Quinine.

Storage conditions

The drug should be stored out of the reach of children, dry, protected from light at a temperature of no higher than 25 ° C. Shelf life is 5 years.

Against the background of taking the drug, radiopaque substances, colloidal blood substitutes and penicillin should not be used.

With prolonged (more than 7 days) use of the drug, it is necessary to control the picture of peripheral blood.

The drug should not be used for acute abdominal pain (until the cause is clarified).

In case of side effects (including those not mentioned above), the patient should stop taking the drug and consult a doctor.

Analgin-Quinine can be used simultaneously with antibiotics, chemotherapeutic agents and vitamins.

In patients suffering from bronchial asthma and pollinosis, the development of hypersensitivity reactions is possible.

Influence on the ability to drive vehicles and control mechanisms

During the period of taking the drug, you should not engage in driving vehicles and work with mechanisms, as there is a risk of reduced ability to concentrate and speed psychomotor reactions.

Overdose

Symptoms: nausea, vomiting (including bloody), melena, epigastric pain, agranulocytosis, aplastic anemia, hemorrhagic diathesis, dizziness, tinnitus, clonic and tonic seizures, cardiotoxic effect, skin rash (bullous, petechial, - or typhoid-like), anaphylactic shock, coma is possible.

Treatment: gastric lavage, the appointment of laxatives and gastroprotective drugs, conducting forced diuresis, breathing, anti-shock measures, the restoration of BCC; if necessary, symptomatic therapy, with the development of seizures - iv barbiturates and diazepam. There is no specific antidote.
